Why am I not losing weight even with semaglutide

You started semaglutide. You did everything right. And the scale will not move.

First: You Are Not Broken

This is one of the most common things patients tell us at Made Ya Skinny. A stall does not mean the treatment failed. It usually means something else in your body is getting in the way.

Weight loss is never just one switch. It’s food, sleep, hormones, stress, dose, and how long you’ve been at it. When one of those is off, progress slows down.

Let’s walk through the real reasons this happens.

Reason 1: You May Still Be Early In The Dose Schedule

Most GLP-1 plans start low on purpose. The starting dose is a warm-up. It helps your stomach get used to the medicine.

Research suggests the lower starting doses are mostly about safety and tolerance, not results. Many patients report that things shift once they move up under a provider’s care.

What to do

  • Check where you are in your plan
  • Ask your provider if it’s time to step up
  • Never change your own dose

Reason 2: You Are Not Eating Enough Protein

This one surprises people. GLP-1 medicine lowers appetite. That’s the point. But if you eat very little, you can lose muscle along with fat.

Muscle burns calories all day long. Lose muscle, and your engine runs slower. The scale stalls even though you’re barely eating.

What to do

  • Aim for protein at every meal
  • Eat protein first on the plate
  • Try a shake on days when food feels hard

Reason 3: Your Hormones Are Working Against You

This is the piece most weight loss clinics skip entirely.

Your thyroid sets your metabolism speed. Low thyroid can slow everything down. Low testosterone in men is linked with more belly fat and less muscle. In women, perimenopause and menopause shift where the body stores fat.

Cortisol — your stress hormone — matters too. High stress and poor sleep can keep cortisol up, and research suggests that pattern makes fat loss harder around the middle.

If your hormones are off, you can do everything else right and still feel stuck.

What to do

  • Get real labs — thyroid, sex hormones, fasting insulin, A1c
  • Ask whether hormone optimization or BHRT makes sense for you
  • Treat sleep like medicine, not a luxury

Reason 4: You Are In A Normal Plateau

Bodies adjust. As you lose weight, a smaller body needs fewer calories. What worked at the start may not be enough now.

Also, the scale lies on any single day. Salt, water, hormones, and workouts all move that number. Patients report going four weeks flat and then dropping several pounds in one week.

What to do

  • Weigh weekly, same day, same time
  • Track waist inches and how clothes fit
  • Add strength training two or three days a week

Reason 5: Hidden Calories And Liquid Calories

Appetite is down, so eating feels small. But sweet coffee drinks, juice, alcohol, and “healthy” snacks add up fast — and they don’t fill you up.

Log everything you eat and drink for seven days. Just seven. Most people find something they forgot about.

Reason 6: Sleep, Stress, And Alcohol

Short sleep raises hunger hormones. Stress raises cortisol. Alcohol adds calories and wrecks sleep quality on top of it.

You cannot out-medicate a body that never rests. Seven hours is the floor, not the goal.

Reason 7: Nobody Is Actually Watching Your Case

A lot of online programs mail you a vial and disappear. No labs. No check-ins. No adjustments.

Medical weight loss is supposed to be medical. That means someone reviews your bloodwork, watches your progress, and changes the plan when it stops working.

If no one has looked at your labs in six months, that’s not a treatment plan. That’s a subscription.

When To Get Re-Evaluated

  • Four or more weeks with no change in weight or inches
  • You feel tired, foggy, or cold all the time
  • Side effects are making the plan hard to follow
  • You have never had full labs drawn
  • Your program has no provider you can actually reach
